Six parts, four real de-identified notes coded live, and the payer questions practices are asking right now.
The core rules from the session, in the order we taught them. Watch the recording for the full walk-through and the reasoning behind each answer.
Every prenatal visit is its own claim, leveled on what happened at that visit.
One code per calendar day of labor: initial or subsequent, straightforward or complex.
The delivery event only. Placenta and 1st or 2nd degree repair are included.
Each hospital day after the delivery date, plus each office visit.
To bill 99214, the note must also show moderate data (3 items) or moderate risk, such as a prescription decision. If nothing was reviewed, ordered, or changed, the visit is a 99212, even for a high-risk patient.
A CBC or lab panel is one item, even if it is listed under three diagnoses. Results of tests ordered at an earlier visit were counted then. An ultrasound your practice bills with its own interpretation does not count.
Several visits by different providers on one date are one service, billed at the highest level reached that day. A previous cesarean, more than one fetus, or a fetal heart rate problem that changes management makes the day complex.
Care on the delivery date is included in the delivery code. A visit at 1 AM the next day is billable (99231 to 99233), and discharge is 99238 or 99239 by time.
Note 1: long high-risk problem list, urinalysis ordered. Note 2: urinalysis and screening ultrasound ordered. Note 3: CBC, glucose screen, RPR, and urinalysis ordered, each listed under three diagnoses.
Tuesday evening: admission, misoprostol, Category I tracing. Wednesday: Category II tracing, oxytocin stopped and restarted lower after intervention, then a spontaneous vaginal delivery with a second-degree repair.
One prior low transverse cesarean. Admitted in spontaneous labor, augmented with oxytocin, arrest of dilation at 6 cm, repeat cesarean the same day.
Vaginal delivery Monday 10:48 PM. Visits: Monday 11:30 PM, Tuesday morning, Wednesday discharge (25 minutes), and a day-12 office visit that orders three tests and starts nifedipine for new postpartum hypertension.
If most prenatal visits land at 99212 or 99213, one vaginal episode comes in about 11% below the old global code.
The gap closes when notes show the work that was actually done, not by coding every visit 99214.
Work RVUs only. 59400 = 37.00 (2026). New values: AMA/RUC recommendations (Feb 2026). Assumes 12 prenatal visits (1 new, 11 established), 2 labor days (59080 + 59082), 59431, and postpartum 99232 + 99238 + one office 99213. CMS July 2026 proposed rule would raise labor and delivery values about 15%; final values expected November 2026.
