What We Hear Before a Change

## The Concerns Practices Bring Us, and How We Handle Each One

Most practices that hesitate have a good reason. Often it is a transition that went badly once. Here is what we hear most, and what we do about it.

About cash flow“We will lose money during the switch.”

#### How We Handle It

Money gets lost in the handoff, not in the decision to move. So we plan the handoff first, with an owner and a date for each step.

- Payer enrollment checked for every provider before anything else changes
- Remittance and deposit routing mapped, then the first payments verified
- Charge lag and denial rate reviewed every week for the first 90 days

#### What It Looks Like

Before go-live, the practice gets a one-page plan listing each step, who owns it and the date it is confirmed. If a payer enrollment is still pending, the start date moves, not the cash.

About systems“We just changed our EMR. We cannot change systems again.”

#### How We Handle It

Your EMR and your billing partner are two separate decisions. Before anything starts, we map which systems stay, which logins change and what each person does differently, and you see it in writing.

- No system change is assumed
- Any change to logins or workflows is listed before you agree to it
- Front desk steps are reviewed with your team, not handed to them

#### What It Looks Like

A practice that just went through an EMR move sees the full list of what changes for their team before signing anything. If the list is longer than they want, they know that up front.

About your team“Our staff and providers cannot handle another change.”

#### How We Handle It

Change fatigue is real. In a well-run transition, most of the work sits on the billing side, not with your team.

- Front desk trained on anything that changes at check-in before go-live, not after
- Providers get a short list of what, if anything, changes for them
- One point of contact for your manager during the first 90 days

#### What It Looks Like

For most providers, the day after go-live looks like the day before. The front desk gets a short walkthrough of any changed steps the week before, and a name to call.

About old claims“Our old A/R will fall through the cracks.”

#### How We Handle It

Claims already in flight on the day of the change are the money most at risk. We treat them as their own project.

- A clean cutover by date of service, so ownership of every claim is clear
- A named owner and a timeline for old A/R
- A weekly old A/R report until it is worked down

#### What It Looks Like

Visits before the cutover date stay with the existing process. Visits on or after it come to us. Old A/R gets its own weekly report, separate from new claims, so it never competes for attention.

About know-how“Our biller knows everything about our practice.”

#### How We Handle It

That knowledge is valuable, and it should not live only in one person’s head. We write it down as part of the handoff.

- Payer quirks, workarounds and fee schedule notes collected before go-live
- Practice-specific rules turned into written workflows
- Your team keeps a copy

#### What It Looks Like

During onboarding we ask the questions that usually only come up when a claim denies: which payer needs what, which provider documents how, which workaround is in place and why.

About patients“Our patients will get confused statements.”

#### How We Handle It

Patients notice a transition through their statements. We decide that part ahead of time.

- A clear plan for who sends statements for old dates of service
- A one-line answer for the front desk when a patient calls
- Patient balances tracked on their own during the change

#### What It Looks Like

The front desk has a short script ready for the patient who calls about two statements, so the answer is the same every time.

About timing“Our numbers look fine right now.”

#### How We Handle It

They might be. One strong month can also hide aging claims or a payer catching up on a backlog. We suggest checking the trend before deciding anything, either way.

- Score the six numbers above with a three-month average
- Look at the reds before the total
- Score again next month and compare

#### What It Looks Like

A practice with a record month and a rising A/R over 90 sees both on the same page, and decides from the trend instead of the deposit.

### The Order We Follow in Every Transition

**01 Credentialing**Every provider enrolled with every major payer, started first.

**02 Payment Routing**Remittances and deposits mapped, first payments verified.

**03 Clean Cutover**One date of service decides who owns every claim.

**04 Old A/R Project**Its own owner, timeline and weekly report.

**05 First 90 Days**Charge lag and denials reviewed weekly, not monthly.

From Dr. Signorelli

## Two Things Practices Tell Us Before a Billing Change

Most of the fear around a billing change comes from a transition that went badly. Dr. Heather Signorelli, our physician co-founder, talks through the two concerns we hear most.

- "We will lose money during the switch."
- "Our staff and providers cannot handle another change."

Who It Is For

## For Practices With Doubts That Are Not Ready to Change Anything

### Practice Owners

You are not sure billing is working, but one strong month keeps pushing the question to next quarter.

### Practice Managers

You hear "billing is not working" from one side and "everything is fine" from the other. Numbers get you out of the middle.

### Practices Burned by a Past Switch

A transition went badly once and nobody wants to repeat it. This lets you check before deciding anything.

Why Six Numbers

## One Good Month Is Not a Healthy Revenue Cycle

A strong month of collections tells you what came in. It does not tell you what should have come in, what is stuck, or what is getting older. It can be a payer catching up on a backlog or a batch of appeals finally paying.

Six numbers, watched every month in the same order, show the trend. Then the decision to stay, fix or move is based on evidence.
